New Patient Information

To facilitate your initial appointment please do the following:

1. Please call one of our convenient office locations to schedule your consultation and have your dental and medical insurance information available to provide to us by phone.
2. Print Health History form (click here), complete the form in entirety and bring with you to your initial appointment. 
3. Bring a complete list of your current medications to your initial appointment.
4. Are you currently under the care of a physician? Please provide us with the names and phone numbers.
